Anzai-gumi is shaken when Wakagashira Akira Kawabe (Yoshiyuki Yamaguchi) is attacked. Learning of the Nakahama-gumi’s involvement, boss Takashi Anzai (Koichi Iwaki) heads there alone. He confronts Nakahama-gumi leader Hiroshiro Matsukaze (Shirō Shimomoto), recently released from prison. Despite his long-standing respect for Matsukaze, Anzai reports the attack on Kawabe and demands the Nakahama-gumi take responsibility. Shocked, Matsukaze pleads for forgiveness, asking Anzai to let it go for his sake... but Anzai won’t back down.
Anzai Takashi (Koichi Iwaki), the leader of the Anzai-gumi, enjoys poker at the Kishochokai’s underground casino. Meanwhile, a heated poker duel unfolds between female gambler Himiko (Sayaka Tsuruta) and university student Kota Katori (Shota Haruta). Kota loses, wiping out ¥2 million. Tokunaga (Masami Tate), a half-gangster, schemes to clear his ¥5 million debt by using Kota in a rigged game but gets caught, increasing his debt to ¥7 million. The next day, Tokunaga disappears, leaving Kota to shoulder the debt. Desperate, Kota’s girlfriend, Anna (Mari Yamaji), returns to her family home and explains the situation to her mother, Reiko. Seeking help, Reiko turns to her husband, who is none other than Kawabe Akira (Yoshiyuki Yamaguchi), the Anzai-gumi’s second-in-command.
Anzai-gumi boss, Anzai Takashi (Koichi Iwaki), visits a jazz club run by corporate affiliate Shin-kai Yuji (Hitoshi Ozawa) to hear pianist Namura Miyako’s (Ako) performance. Miyako, who had been supported by Anzai since her youth, harbors deep feelings for him. One day, Mishima (Masaki Nishimori) from the Nakahama-gumi visits the club with a woman and becomes infatuated with Miyako, aggressively pursuing her. Despite her initial reluctance, Miyako gradually falls for Mishima. Soon after, her behavior becomes increasingly strange. Sensing something amiss, Anzai orders his men to investigate Miyako’s actions, leading to unexpected revelations...
Six years have passed since Tetsuo Sakuragi (Yasukaze Motomiya) became the Wakagashira of the Shimazu Clan. One day, Sakuragi becomes the target of an assassination attempt by Mamoru, the son of the disbanded Morita Clan's leader. Though Sakuragi is shot, his injuries are not life-threatening. Understanding Mamoru’s dire circumstances, Sakuragi chooses to let the matter slide. However, the incident throws the Shimazu Clan's headquarters into chaos. Meanwhile, Hidetoshi Kaji (Hitoshi Ozawa), the leader of the Koga Family, interrogates Shimazu Clan member Takuzo Katsuya (Yoshimi Tachi) and learns about Mamoru’s involvement. Determined to settle the score, Kaji storms the Matsuno Association, the group that inherited the Morita Clan’s remnants, but fails to locate Mamoru. Relentless, Kaji has Matsuno’s members monitored, tracks down Mamoru’s whereabouts, and ultimately abducts him...

To celebrate his 30th acting anniversary, Kōji Matoba stars as Seiji Oda, the fiercest member of Tendokai, who joins the organization under Kazuma Washio (Hitoshi Ozawa). In 2011, Tendokai dominates Kanto, while the rising Santōkai threatens them. Meanwhile, in Yokohama, Seiji Oda forms a rogue group, Oda Dōshi-kai, and violently protects his turf, clashing with Jinseikai. As Tendokai's Vice-Head Ryuji Kondo retires, Keiji Oda (Seiji's brother) is tasked with calming the tensions between the two groups, leading to a reunion between the Oda brothers.
Yasuda (Hitomi Matsuda), the Wakagashira of the Takemiya Union Tōjō-gumi, infiltrates the hideout of the Yamashina-gumi's Sakazaki (Junichi Kawamoto), who had been secretly engaged in organ trafficking. Yasuda tries to extract the truth from Sakazaki, but is killed by Detective Hyūga from the Organized Crime Division (Marubō). The blame is pinned on Yasuda, leading to his arrest. Two years later... In Yasuda's absence, the boss of the Tōjō-gumi, Tōjō (Hiroyuki Watanabe), is assassinated by a hitman from the Myōjin-kai. The situation changes drastically. Nikaidō (Tarō Kawano) pressures Yasuda to reestablish his oath of allegiance, but Yasuda firmly refuses. Frustrated by Yasuda's response, Nikaidō makes a certain proposal to the union's chairman, Shimamura (Shunsuke Kariya).
